What is Janumet XR used for?
JANUMET XR (JAN-you-met) is a prescription drug that contains 2 prescription diabetes medicines, sitagliptin (JANUVIA®) and extended-release metformin.
JANUMET XR can be used along with diet and exercise to lower blood sugar in adults with type 2 diabetes.
This drug targets 3 main factors to help lower blood sugar levels:
- Helps your pancreas make more insulin
- Helping to decrease excess sugar that your liver makes
- Enables your body use the insulin that it makes more effectively
- Helps control blood sugar throughout the day, including after meals
- Is not likely to cause weight gain and low blood sugar (hypoglycemia) by itself
Type 2 diabetes mellitus
Type 2 diabetes mellitus occurs when your body does not make enough insulin and the insulin that your body produces does not work as well as it should.
Your body can also make too much sugar. If this happens, sugar (glucose) levels build up in the blood which result in high blood sugar (hyperglycaemia).

All doses contain: povidone, hypromellose, colloidal silicon dioxide, sodium stearyl fumarate, propyl gallate, polyethylene glycol, and kaolin. Film coating contains hypromellose, hydroxypropyl cellulose, titanium dioxide, FD&C #2/Indigo Carmine Aluminum Lake and carnauba wax.
In addition the JANUMET XR 50 mg/1000 mg Tablets also contain: yellow iron oxide.
How does this medication work?
This medication contains two active ingredients, sitagliptin (as phosphate monohydrate) and metformin hydrochloride.
Sitagliptin belongs to a class of medicines called DPP-4 inhibitors (dipeptidyl peptidase-4 inhibitors), and metformin belongs to a class of medicines called biguanides.
Sitagliptin and metformin work together to control blood sugar levels in patients with type 2 diabetes mellitus. This means Janumet XR:
- helps to increase insulin levels after a meal.
- lowers the amount of sugar made by the body.
- works when blood sugar is high, especially after a meal. This is when the body needs the greatest amount of help in lowering blood sugar.
- also lowers blood sugar between meals.
How to take
Take only as and when prescribed by your doctor.
Your doctor will tell you how many JANUMET XR tablets to take. They should be taken once a day.
Take them with meals to lower your chance of an upset stomach.
Swallow the tablets whole with a glass of water. Do not chew, cut, or crush the tablets.
Tell your doctor if you cannot swallow these whole. You may see something that looks like the JANUMET XR tablet in your stool (bowel movement).
If you see tablets in your stool several times, talk to your doctor.
Do not stop taking this medication without talking to your doctor.
Follow all directions given to you by your doctor and pharmacist carefully.

How long should you take this medication?

Missing a dose

Before you take this medication, tell your doctor if you:

– have or have had inflammation of your pancreas (pancreatitis).
– are going to get an injection of dye or contrast agents for an x-ray procedure
– have severe kidney problems.
– have liver problems.
– are pregnant or plan to become pregnant.
– have heart problems, including congestive heart failure.
– have any other medical conditions.
It is not known if JANUMET XR will harm your unborn baby. If you are pregnant, talk with your doctor about the best way to control your blood sugar while you are pregnant.
